The XHTML demo database is part of the Domino XHTML Forms article. You can use the following database as a reference or example to study the article. It holds many other interesting web techniques, so feel free to peek around.
You can also deploy the demo database on your own server. Simply deploy and browse to the nsf in your browser. Note that by default manager access is given for demo purposes.
